3. You will receive an email from our team confirming receipt.

Our team will verify your insurance on your behalf and provide you with a detailed explanation of benefits via email.
Required Documentation for Scheduling
To schedule the initial evaluation or first appointment, we will need:
1. completed New Patient Packet
2. Finalized signed copy of IEP (Individualized Education Plan) or IFSP (Individualized Family Service Plan), if applicable
3. Hearing screening results (required for PeachState Health and Amerigroup patients)
