To get started with Chatterbox Inc.

  1. Download and complete our New Patient Packet. Once completed,

  2. Email the packet to info.chatterboxinc@gmail.com or fax it to 770.569.7432. You will receive a confirmation email once we have received your packet.

  3. Our team will verify your insurance and send you a detailed explanation of benefits.

  4. To schedule the initial evaluation or first appointment, we will need the completed New Patient Packet, a finalized signed copy of your IEP (Individualized Education Plan) or IFSP (Individualized Family Service Plan), if applicable, and hearing screening results (required for PeachState Health and Amerigroup patients).

  5. Once we have all necessary documents, we will schedule your evaluation. If you have any questions or need assistance, feel free to reach out!
